UPDATE: VHA Mandates vaccination for Title 38 employees

UPDATE: VHA has mandated ALL Title 38 employees be vaccinated. However, there are exemptions. ANY employee may request an exemption under the exemption criteria Medical or Religious reasons which is the same process for the flu vaccine.
(A) Medical Exemption should be requested by completeing Covid Vaccination Form VA Form 10-263 (Appendix B) declaring an exemption for medical contraindication with a personal physician signature with their National Provider Identification number. The contraindication must be one recognized by the Centers for Disease Control and Prevention found here: https://www.cdc.gov/vaccines/covid-19/clinical-considerations/covid-19-vaccines-us.html?CDC_AA_refVal=https%3A%2Fwww.cdc.gov%2Fvaccines%2Fcovid-19%2info-by-product%2Fclinical-considerations.html VHA Title 38 employee must obtain their immediate supervisor's signature WITHOUT disclosing the medical reason. The employee must submit the completed VA Form 10-263 (Appendix B) to the Employee Occupational Health staff within 8 weeks of publication.
(B) Religious Exemption If the Title 38 employee declines to recieve the COVID-19 vaccine due to a deeply held religious belief, they must complete the COVID-19 Vaccination Form 10-263 (Appendix B) declaring exemption for deeply held religious belief. The Title 38 employee must obtain immediate supervisor signature on the VA Form 10-263 (Appendix B) WITHOUT disclosing the religious reason to their supervisor. The employee must submit the completed VA Form 10-263 (Appendix B) to the Employee Occupational Health staff within 8 weeks of publication.
